Eligible students may also insure their dependents. Eligible dependents are the spouse/same-sex domestic partner (residing with the Insured
student) and unmarried children under the age of 24. Eligibility may also be extended, upon approval by the plan, beyond the limiting age for
children who are disabled if the child was included as a dependent at the time of initial enrollment of the employee. Newborn children of an
eligible child are eligible for coverage for up to 18 months after birth. Eligible dependents are the insured's spouse (residing with the insured
student) and unmarried children under 19 years of age, who are not self-supporting and who reside with the insured student. Dependent eligibility
expires concurrently with that of the insured student.
Eligibility ends when you fail to register for a semester. Coverage ends for your dependents, if you have enrolled them (see above right),
when your coverage ends or their eligibility ends. Eligible students who enroll in the plan may also insure their dependants. Students must
actively attend classes for at least the first 31 days after the date for which coverage is purchased to remain eligible.

Students withdrawing after 31 days will remain covered under the SHIP for the full period
for which premium has been billed. No refund will be allowed. Students are strongly urged to bring an emergency credit card to pay for medical
services. Claim forms can be printed from the EAP website. Students must pay their bill in full to register.
Students should be familiar with their coverage so that they can specify particular labs or pharmacies if necessary. The Health Services staff
may help identify local resources that meet the requirements of individual managed care plans. Students are encouraged to contact the school for
more exact estimates. The actual costs of your education can vary based on the university your choose to attend, credits you are transferring in,
and the way you choose to finance your education. Students from other provinces are usually covered under their own provincial plans, but
regulations vary from province to province. Some provinces require students attending out of province universities to submit a letter of
acceptance to the university as well as proof of registration for the current year.
Students must be registered for the semester they wish to reinstate, and they must be eligible to be assessed the fee. Change of status
reinstatement can be done throughout the semester, based on the termination of your other health insurance. Students must complete an Enrollment
Form (to be provided) and remit payment for selected coverage (Student only, Student & Spouse, etc.). Credit cards cannot be accepted.
Students can choose to keep SHIP, or they can waive enrollment if they have comparable coverage.
Students who have not received their insurance ID cards may obtain a prescription from a preferred Aetna Pharmacy by having the pharmacist
contact Aetna Pharmacy Management at 800.238.6279 to verify that he/she is covered. Students not yet updated in Aetna’s system must purchase the
prescription and submit the receipt and prescription stub with a completed Aetna Pharmacy Drug Claim Form to be reimbursed for covered
medications directly from Aetna. Student Health Center will no longer directly bill insurance for services rendered to GSHIP members. GSHIP
members using Student Health Center's Medical or Mental Health services will pay for the service at the time of service. Student Secure comes in
two levels, budget and select. The plan provides comprehensive inpatient and outpatient medical coverage up to $250,000 (budget) or $300,000
(select).
Students who pay the billed premium are automatically enrolled in the Student Health Insurance Plan unless a waiver is submitted and approved
of comparable or better alternate health coverage. As stated above, eligible graduate students will receive a subsidy of $375 per semester to
help defray the cost of the insurance premium for the student. Students must be enrolled at Indiana University for at least the first 31 days
after the date for which coverage is purchased. Home study, correspondence, internet classes (with the exception of Nursing PhD candidates) and
television courses do not fulfill the eligibility requirements.
Students should know the name of their plan, the insurer’s address and the policy number, and should carry insurance cards with them. Student
insurance is not automatically added to bills except for International Students - see next paragraph. Students who have other health insurance
must sign a waiver at the business office if they do not wish to carry the student health insurance. Student charges generated at University
Health Center may be paid at the time of service or forwarded to Student Accounts. They will appear on the student's monthly billing statement as
Health Services.

Dependent coverage not available for undergrads. Depending on your personal circumstances, some health insurance companies' rates may be less
than others. During your job search, it's a good idea to consult any prospective employers on what their health benefits are; since some
employers will provide more health insurance coverage then others. Depending upon your earnings, even a working family can obtain the health
insurance they require. However, in most cases adults can also meet up the requirements with co-pay.
Dependents can be added to your policy for an increase in premium. They will not, however, have access to care at Cutler and the University of
Maine will not cover 50% of their premium cost. Dependent children of any age who are disabled can be covered by the their parent's policy.
Depending on your policy, some services are not included, like cosmetic dentistry or mental health care. And some items are limited, such as the
length of time you can stay in the hospital.
